Theme: Glaucoma

Materials on the topic of the practical class prepared assistant of the Department of ophthalmology  Voronova N.N.

Glaucoma is rather common disease, particularly among old-age persons; but the disease also infrequently affects people who are able to work. Up to 15% of patients with glaucoma are disabled for vision, which makes the problem of diagnosis and treatment of glaucoma to be urgent.

Knowledge of clinical manifestations of glaucoma enables to diagnose timely the disease, to estimate properly the state of a patient, and, in case of emergency, to render first aid at acute attack of glaucoma. Early diagnosis and timely administered treatment make the prognosis to be more favourable, and promote decrease of disability at given disease.

Purpose of Training:

On the basis of current concept about the etiopathogenesis of glaucoma, and evaluation of clinical and biochemical and instrumental data, to learn to diagnose this disease, to render timely and skilled assistance in case of acute attack of glaucoma, to administer proper treatment taking into consideration the peculiarities of the course of the disease in each and every case, and to facilitate complete social rehabilitation.

Student should know:

- etiology and pathogenesis of glaucoma;

- clinical course of open angle glaucoma, closed angle glaucoma, and congenital glaucoma;

- complaints and objective symptoms of glaucoma;

- methods of examination of patients with glaucoma;

- peculiarities of changes of the visual fields as one of the main signs of glaucoma;

- clinical signs of acute attack of glaucoma;

- fundamental principles of emergency care in case of acute attack of glaucoma;

- peculiarities of treatment of patients with glaucoma at the in-patient department;

- indications for surgery in patients with glaucoma;

- the essence of step-wise treatment of patients with glaucoma, and in case of attack of glaucoma;

- prognosis for rehabilitation of vision;

- prophylactic measures.

Student should be able to:

- to listen and estimate properly the patient’s complaints, to take the case

history and objective findings;

- to formulate accurately an initial comprehensive clinical diagnosis of the

diseases in each patient;

- to plan the examination of the patient to confirm the diagnosis;

- to estimate the visual field in each patient;

- to measure the intraocular pressure;

- to render first aid the patient with acute attack of glaucoma;

- to make differential diagnosis of acute glaucoma against keratitis and iridocyclitis;

- to administer an adequate treatment to the patient with primary glaucoma;

- to differentiate the signs of secondary glaucoma;

- to interpret the laboratory, instrumental, and additional methods of investigation;

- to make a final comprehensive clinical diagnosis;

- to plan the treatment of each patient taking into consideration the final

comprehensive clinical diagnosis (with due regard to the clinical features

of the disease);

- to plan the follow-up treatment of the patient and his social rehabilitation;

- to make the prognosis of the disease.
