Theme 6: Diseases of the lens

Materials on the topic of the practical class prepared assistant of the Department of ophthalmology Voronova N.N.

Task 1.

The patient aged 70 has noted gradual diminution of vision in the right eye within the last year. Objectively: visual acuity in the right eye is 0,5. The eye is quiet. IOP is 22 mm Hg. There are pin-like opacities of gray colour on the periphery of the lens. It is noted angiosclerosis of the fundus.

1. Make the diagnosis:

a) primary glaucoma;

b) uveitis;

c) senile cataract;

d) optic atrophy;

e) maculodystrophy (degeneration of the macula).

Task 2.

Parents of the child, whose age was 6 months, noticed opacity in the pupil. Objectively: the eye is quiet; corneal diameter is 9 mm; centrally, on the anterior capsule of the lens, there is a minute opacity; fundus of the eye does not have any pathology.

1. Make the diagnosis:

a) congenital glaucoma;

b) microcornea;

c) macrocornea;

d) congenital cataract;

e) optic atrophy.

Task 3.

Within the last three months the female patient aged 55 had gradually developed significant diminution of vision in her left eye. That day there appeared severe pain and redness of the eye. Objectively: visual acuity in the left eye is 0,02 n/k; congestive hyperemia of the eyeball; the tone of the eye is sharply increased; the anterior chamber is shallow, its contents are transparent; the lens is diffusively turbid; fundus reflex is absent.

1. Make the diagnosis:

a) acute iridocyclitis

b) keratitis

c) acute attack of secondary glaucoma, swelling cataract

d) acute attack of primary glaucoma

e) conjunctivitis

Task 4

The patient is 70 years old. VОD = photoperception from the correct projection. VОS = 0.05 n/c. In examination: ОД – christalline lense is completely dull, no shades from the iris, reflex of the fundus of the eye is absent. IOP = 20 mm/Hg; ОС – inferior chamber is shallow, christalline lens is grey, increased in size, Many transparent and half-transparent fibers, reflex of the fundus of the eye is very weakened, IOP = 20 mm/Hg.
