Task 3.

A patient developed a local tenderness in the right eyeball near the external border of the palpebral fissure simultaneously with exacerbation of rheumatism. Objectively: eyelids are edematous; lacrimation; local hyperemia of the mucous coat near the external border of the palpebral fissure and infiltrate of purple colour in this area; tenderness is noted on palpation of the eyeball. Refractive media are transparent.

Task 4.

A week ago a patient aged 35, has developed edema and redness of the right eyelid, bad headache, and increase of temperature up to 38,5ºC. Within the previous 5 days the patient has been treating by an ENT doctor for exacerbation of right hemisinusutus, which he suffers for many years. Objective studies have revealed: high degree exophthalmoses, chemosis, the absence of motility in the right eye, and the symptoms of right purulent hemisinusitis.

Which rhinogenous complication may be supposed in this case?

1. Phlegmon of the orbit ?

2. Edema of the eyelids ?

3. Periostitis of the orbital walls?

4. Abscess of the eyelids?

5. Retrobulbar abscess? 

Prescribe treatment.

Task 5

A patient experiences difficult nasal breathing; there is purulent discharge from the nose. Pain in the right orbit and protrusion of the eyeball has appeared two days before. Objectively: visual acuity in both eyes is within normal limits. There take place hyperemia and edema of both eyelids, limitation in ocular movements, and pain. Moderate exophthalmos in the right eye. Refractive media are transparent. Fundus is normal.
